Family medicine is the medical specialty that provides continuing and comprehensive health care for the individual and family. The scope of family practice encompasses all ages and genders, each organ system and every disease entity.
We are a part of the Fairfax Family Practice Centers, which have many offices throughout Northern Virginia. All of our physicians are dedicated to providing medical care that is specifically tailored to your needs and the needs of your family.
Family Medicine is designated a Level 3 Patient-Centered Medical Home by the National Committee for Quality Assurance (NCQA). This innovative program to improve primary care encourages and facilitates partnerships between individual patients and personal physicians and, when appropriate, the patient's family. FFPC is organized as a physician-led care team that provides for every patient's healthcare needs and coordinates care across the healthcare system.
Fairfax Family Practice Centers is a member of the HealthConnect IPA
HealthConnect IPA is an organization created to build and manage an independent physician network in Northern Virginia. This network of providers is dedicated to providing clinically integrated medical and health care services that are quality based, value driven, and patient centered, achieving “Triple-Aim” Goals.
The Triple Aim initiative is a framework developed by the Institute of Healthcare Improvement (IHI) that focuses on optimizing health system performance through the pursuit of three dimensions:
- Improving the patient experience of care (including quality and satisfaction);
- Improving the health of populations; and
- Reducing the per capita cost of health care.
